Please note:

If you order a replacement card your current card will be cancelled. You should only order a replacement card if your current card is lost or stolen.
You do not need to order a replacement card if you have changed your membership or level or cover.
Never attempt to use a cancelled membership card with a HICAPS provider as this will not be accepted, and your claim will be rejected.
