a replacement or additional memebrship card and we will send
the card to you within the next 3 business days.
Please
note:
- If you order a replacement card your current card will
be cancelled. You should only order a replacement card if
your current card is lost or stolen.
- You do not need to order a replacement card if you have
changed your membership or level or cover.
- Never attempt to use a cancelled membership card with
a HICAPS provider as this will not be accepted, and your
claim will be rejected.
